There are two separate processes you must complete to come back to SJSU as a regularly matriculated student:

1. Reinstatement
2. Readmission

Reinstatement

Follow this step by step guide to become reinstated to SJSU:

1. Download reinstatement form (Reinstatement form for Graduate students)
2. Write a personal statement
3. Attach supporting documentation to petition
4. Make an appointment with your major advisor or department chair
5. Turn in the completed and signed petition form and personal statement to window “R” in the SSC lobby

Readmission

1. Reapply to SJSU for the next available semester at http://csumentor.edu
