Instructions for filling out the major form

Download the major form. You need to type in the form, and no handwriting will be accepted. There isn't much room to spare in the left and right margins, so don't make the columns much wider than they already are.

Next to each course listing, write the grade you received (e.g. A-). Be accurate! If you took the course more than one, write all of the grades you received separated by slashes (e.g. D/B+).

If you took an equivalent course at another college or university, then strike through the course on the form and write the course you took instead in the space below. Footnote the course name and in the Notes section write the name of the college or university where you took the course (e.g. 1. Foothill, 2. CSULB). If the equivalent course was actually two courses (e.g. the school where you took it was on the quareter system) then list both courses separated by slashes, both unit values seprated by slashes, and both grades, separated by slashes. You can also use this technique for AP courses taken in high school. For example, strike through Calculus I and in the space below write "AP Calculus". Equivalent courses taken at SJSU don't need to be footnoted. For example, many students will strike through CmpE120 and write below CS147.

In the spaces below each elective cell (Elective 1, 2, 3) enter the elective course you took or will take. This must either be a pre-aproved elective or one aproved by the BSSE advisor.

After the form is filled out correctly, print it out, sign it, and bring it to the BSSE advisor for a signature. The form then goes to the CmpE office where it will be checked for accuracy before forwarding it to the registrar.
