How to Renew an Emergency Permit

  1. Obtain from your employer form CL-469. Your employer should fill out the upper half of this form.

  2. Make a copy of your most recent emergency permit even if it has expired.

  3. Log on to https://my.sjsu.edu. Navigate to the Account Summary page to view grades. Print out a copy of your grades that shows 6 semester units of coursework completed toward your credential since the issue date on your expiring emergency permit. If you did your coursework at another university please provide a copy of transcript showing your grades.

  4. FAX to 408-924-7061 or mail all of the above copies to:
    • San Jose State University
      Credential Services
      Sweeney Hall Rm 107
      San Jose, CA 95192-0015

  5. Please provide your:home address, daytime telephone number and email address.

  6. We will mail you the original approved CL-469 form as you will have to register it in person at your County Office of Education.
