Request for Equipment/Lab Form

Please complete this form to request for Equipment or Lab usage.
Please follow these steps:

Step 1:
Check the updated calendar at www.ical.sjsu.edu if the item is available, continue to Step 2.

Step 2:
Request for the item or lab by completing the online Request Form below.

Step 3:
Check your email for a COPY of your REQUEST (This is NOT a Confirmation)

Step 4:
Technology Services staff will update the calendar in the order it was received

Step 5:
Technology Services staff will SEND YOU a CONFIRMATION or a DENIAL of your request to the Email Address your provided.

Step 6:
If you do not receive an email from a Tech Staff member within 24 hours, please Forward the REQUEST Email you Received to edutech@sjsu.edu

    If your class is held in a "Smart Classroom" you probably do not need to check-out a media cart. Review the List of Smartclassrooms installed in Sweeney Hall.
