Each Week, you are asked to sit down with your cooperating teacher and ask and answer the following questions:

Current Focus--Challenges--Concerns?

Student Teacher's Next Steps?

Support Provider's Next Steps?

This form is available in your field study guide and is a useful tool in aiding communication with cooperating teachers. It is due each week and can be filled out by colleagues, supervisors, and others such as a librarian, resource teacher . Ask for your cooperating teacher's signature. If time is at a premium, fill it out, and then ask for a signature but ideally this is a time for the two of you to discuss the focus for the week.
