Dear Parent:

As you may know, we have a partnership with San Jose State University Student Teacher Program to place student teachers in some of our classrooms.  Occasionally, the supervisor of the program will take video of the classroom for education purposes.  the student teachers are videotaped teaching lessons and then review the tape to learn from their performance.  In aditionn, videos are shared with other student teachers.

San Jose State University has also requested some photogrpahs of student teachers in the class-room to be used in a slide show for the university's "Convocation Ceremony."

Please indicate below whether or not you give permission for the use of any video or photos of your child obtained in the classroom and used for educational purposes and in a slide show for San Jose State University's Convocation Ceremony on May 13, 2005.

PERMISSION SLIPS MUST BE RETURNED TO YOUR CHILD'S TEACHER BY FRIDAY, APRIL8, 2005.
