Complete and submit this form once you have been awarded financial aid to notify the Financial Aid and Scholarship Office of changes to your enrollment status. This form MUST be completed every academic year and MUST be submitted in person or sent by US postal service.
Parents of dependent students who wish to borrow a PLUS loan must complete and submit this form once the student has been awarded.
Graduate students who wish to borrow a Grad PLUS loan will need to complete and submit this form once they have been awarded and exhausted all Unsubsidized loan limit.
The Student Authorization to Release Information Form authorizes SJSU Financial Aid and Scholarship Office to discuss and/or release financial aid information to the person(s) you designate. The form MUST be completed every academic year and MUST be submitted in person. Students will be asked to show a valid ID when submitting this form.
Review of Extraordinary Expenses [pdf] - 2014-15 form is not available at this time
This form is used to request a review of extraordinary expenses which were not reflected on your 2013-14 financial aid application (FAFSA). Please indicate any extraordinary expenses during 2013 by completing the applicable item(s). A cap of $10,000 worth of expenses can be considered for additional extraordinary expenses beyond one's cost of attendance for each academic year.
GRE Fee Reduction Certificate
ETS offers a limited number of GRE Fee Reduction Certificates. Complete the GRE Fee Reduction Form [pdf] for review of eligibility.
Academic Progress Petitions
The Financial Aid and Scholarship Office is required to monitor academic progress for all students who apply for aid per Federal and State regulations. Click on the Policies link to learn more about Academic progress.
Complete the appropriate petition form based on the hold that has been placed on your financial aid award. To check your hold status go to your "To Do" items in MySJSU .
Academic Progress Petition Form D-Hold [pdf] A hold has been placed on your aid because you have been disqualified from the university.
Academic Progress Petition Form H-Hold [pdf] A hold has been placed on your aid because you have completed less than 75 percent of your total attempted units.
Academic Progress Petition Form U-Hold [pdf] A hold has been placed on your aid because you have excessive units beyond your degree requirement.
