Federal Direct Plus (Parent) Loan

Federal Direct GRAD Plus Loan
2012-2013 Direct Grad PLUS Loan Application [pdf] Graduate students who wish to borrow a Grad PLUS loan will need to complete and submit this form once they have been awarded and exhausted all Unsubsidized loan limit.

Cal Grant Verification Form
Transfer Entitlement Cal Grant Verification Form [pdf] The California Student Aid Commission requires the Financial Aid and Scholarship Office to obtain documentation that verifies you have met eligibility requirements before you may receive your Transfer Entitlement Cal Grant award.
Student Release of Information Form
The Student Release of Information Form [pdf] authorizes SJSU Financial Aid and Scholarship Office to discuss and/or release your financial aid information to the person(s) you designate. The form MUST be returned in person with a valid ID to the Financial Aid and Scholarship Office.
Review of Extraordinary Expenses
2012-2013 Review of Extraordinary Expenses Form [pdf]. Use this form to request a review of extraordinary expenses which were not reflected on your 2012-2013 financial aid application (FAFSA). Please indicate any extraordinary expenses during 2012 by completing the applicable item(s).

GRE Fee Waiver
ETS offers a limited number of GRE Fee Reduction Certificates. Complete the GRE Waiver Form [pdf] for review of eligibility.
Academic Progress Petitions
The Financial Aid and Scholarship Office is required to monitor academic progress for all students who apply for aid per Federal and State regulations. Click on the Policies link to learn more about Academic progress.
Complete the appropriate petition form based on the hold that has been placed on your financial aid award. To check your hold status go to your "To Do" items in MySJSU .
Academic Progress Petition Form D-Hold [pdf] Your overall Academic Progress Status is "D." This means a hold has been placed on your aid because you have been disqualified from the university.
Academic Progress Petition Form H-Hold [pdf] Your overall Academic Progress Status is "H." This means a hold has been placed on your aid because you have completed less than 6 undergraduate/credential/2nd degree or 4 graduate units during the last semester or academic year.
Academic Progress Petition Form S-Hold [pdf] Your overall Academic Progress Status is "S." This means a hold has been placed on your aid because you have excessive semesters beyond your degree requirement.
Academic Progress Petition Form U-Hold [pdf] Your overall Academic Progress Status is "U." This means a hold has been placed on your aid because you have excessive units beyond your degree requirement.
