Phi Kappa Tau (active)

— OCTOBER 20, 2010 Order issued by the Chancellor's Office of the Cal State University System for Misrepresentation, Willful Disregard for University Policy, Several Allegations of Hazing (outlined in an April 23, 2010 letter from the Director of Student Involvement)

— PROBATION following the lifting of a Cease and Desist Order by SJSU and the Chancellor's Office for violation of recognition protocols (operated as an underground group from 2007 – 2013) and improper use of the University's name and image(s), probation from August 1, 2013 through July 31, 2015

Alpha Kappa Alpha

— PROBATION following return from Suspension for hazing, probation from June 11, 2014 through June 10, 2021

— PROBATION for hazing (humiliating or degrading behavior, blindfold activities), beginning November 6, 2014 and through November 6, 2015 — Specific Code Violations: Hazing

Kappa Sigma

— WARNING / PROBATION by the Supreme Executive Committee of the national organization following an unruly social function on April 4, 2014, where chapter funds were used to purchase alcohol, alcohol was present at or following an activity that was part of the Chapter's pledge program, the number of guests exceeded the number of members in attendance, the event included bulk quantities of alcohol, and alcohol was present at a fundraising event for the Chapter — the duration of the Warning / Probation lasts until sanctions are completed

— PROBATION following return from Suspension for hazing, probation from January 1, 2012 through December 31, 2019

Theta Chi

— PROBATION following an investigation into two sexual assaults at the Chapter house, probation from October 8, 2014 through October 8, 2015, as well as a host of educational and operational sanctions to be completed by March 6, 2015

— SUSPENSION of all chapter activities for hazing, beginning February 8, 2009 and through February 7, 2016 — Specific Code Violations: Hazing
