How to File for Graduation

Seniors should file for graduation after they are settled in their classes two semesters before they wish to graduate. For example, a student who wants to graduate in May 2009 should file for graduation in May 2008. Here are the steps for filing for graduation:

  1. Obtain and complete an Application for Graduation. Follow the step-by-step instructions on this form!
  2. Fill out your Major Form in the highlighted areas. Include the semester you took each course and your grade for completed courses. For courses note yet completed, fill in the semester you are going to take the course.
    1. Option 1 (HS General) Major Form is available by clicking here.
    2. Option 2 (HS w/minor) Major Form is available by clicking here.
    3. Option 3 (HS w/Gero) Major Form is available by clicking here.
    4. Option 4 (HS w/Health Admin) Major Form is available by clicking here.
    5. Option 5  (HS w/Health Professions) Major Form is available by clicking here .
  3. Return the completed Major Form to your Advisor for signature. Your Major Advisor will forward the form to the Chair of the department for signature.
  4. In about a week, return to the HS office (MH407) to pick up your signed Major Form.
  5. Take your completed Application for Graduation and Major Form to Student Service Center to complete the process.
