In the event of an accident or injury, the highest priority must always be caring for the individual. Immediately report any work-related injury or illness to your supervisor.

• If the injury is serious, call 911 or contact the University Police Department at 924-2222 and follow their instructions.
• If you can move safely, go to the SJSU Student Health Center for First Aid during the following business hours:

Fall/Spring Semester
Mon/Tues/Thurs: 8:00 - 5:00
Wed: 9:00 - 5:00
Fri: 7:30 - 4:00

Spring/Summer/Winter Break
Mon/Tues/Thurs: 8:00 - 4:30
Wed: 9:00 - 4:30
Fri: 7:30 - 4:00

• The Employee Injury Care Student Health Center Procedures (PDF) provides general procedures for employee work-related injury care at the Student Health Center (SHC).  
• If the injury requires more medical care beyond First Aid, or if the injury occurs while Student Health Center is closed, go to US HealthWorks at:

• If an injury or illness occurs on nights or weekends, go to O’Connor Hospital Emergency Room for initial treatment.

The supervisor / appropriate administrator completes the following two forms. Within 24 hours, fax the forms to HR at 408-924-2144. Send orginals in campus mail to HR/Safety m/s 0046.

• Employer's Report of Occupational Injury or Illness (PDF): Write a description of the accident using the employee's own account of the incident and submit to University Risk & Compliance.
• Workers’ Compensation Claim Form (DWC 1) & Notice of Potential Eligibility (PDF): Provide the injured employee with this form. When the employee completes and returns this form to you, forward it on to University Risk & Compliance.
• Illness & Injury Prevention Program: Investigate the accident and address the problem to prevent future injuries.
