In the event of an accident or injury, the highest priority must always be caring for the individual. Immediately report any work-related injury or illness to your supervisor.

• If the injury is serious, call 911 or contact the University Police Department at 924-2222 and follow their instructions.
• If you can move safely, go to the SJSU Student Health Center.
• If an injury or illness occurs when the Student Health Center is closed, go to the O’Connor Hospital Emergency Room for initial treatment.

Within 24 hours, supervisor / appropriate administrator completes the following:

• Employer's Report of Occupational Injury or Illness (PDF): Write a description of the accident using the employee's own account of the incident and submit to University Risk & Compliance.
• Workers’ Compensation Claim Form (DWC 1) & Notice of Potential Eligibility (PDF): Provide the injured employee with this form. When the employee completes and returns this form to you, forward it on to University Risk & Compliance.
• Illness & Injury Prevention Program: Investigate the accident and address the problem to prevent future injuries.

When treatment is needed beyond the first visit, the Student Health Center or O’C onnor emergency department will refer to appropriate specialist or facility. For additional information, see Workers’ Compensation.
