This year's annual Open Enrollment period begins September 11, 2017 through October 6, 2017. Open Enrollment is your once-a-year opportunity to learn about new offerings and to make additions, changes or deletions to your benefits, which will be effective January 1, 2018. Employees have the opportunity to:
- Enroll in health/dental/vision/flexcash (if benefits eligible)
- Change insurance plans
- Add/delete eligible dependents
- Enroll/re-enroll in Flexible Spending Accounts - Health Care Reimbursement Account(HCRA) and/or Dependent Care Reimbursement Account (DCRA)
- Enroll/cancel voluntary benefit plans
- MetLaw – Legal Plan enroll or cancel (See voluntary benefit plans section below)
CalPERS Online Health Plan Statement
Beginning August 28, 2017, all health plan members can access their Health Plan Statement
by logging into their CalPERS account 24/7 at my|CalPERS. The health plan statement allows subscribers the opportunity to receive customized
employee specific health information in a confidential and secure method and provides
a convenient central location for health plan resources. Employees can view their
current monthly premium rates and 2018 monthly premium rates if they do not change
health plans for Open Enrollment. The statement will also include specific health
benefit changes that may impact them in the upcoming year. If an employee did not
create a CalPERS account, they can create one by going to the my|CalPERS log-in page
located at CalPERS and select Participant, Continue then Register Now.
2017 Employee Benefits and Services Fair
Tuesday September 26, 2017 10:00 a.m. – 2:00 p.m. at the Student Union Ballroom
Human Resources will be hosting our annual Employee Benefits and Services Fair on Tuesday, September 26 from 10:00 a.m. to 2:00 p.m. in the Student Union Ballroom. The fair will offer employees the opportunity to learn more about health plans that will be effective January 1, 2018 as well as local programs that can improve employee health and wellness. Over 70 vendors are expected to attend, including a wide range of health care groups, investment groups, and wellness companies. Fair attendees will be eligible to win door prizes from participating vendors. Bring your Tower Card to be eligible for a variety of giveaways and prizes. We hope to see you at the fair!
Benefits Staff will be on hand at the fair to assist employees with their on-line open enrollment changes.
Benefit elections and changes during Open Enrollment must be made online through MySJSU Self-Service and will take effect on January 1, 2018. You can access it from any computer with Internet access by logging on to your MySJSU and selecting Main Menu > Self Service > Benefits > Open Enrollment. You will need your SJSUOne username and password to log in. Also, please submit an e-Benefits Self Service Electronic Signature Authorization Form (.pdf), if you haven't done so already. All Open Enrollment transactions must be completed in MySJSU by midnight on October 6, 2017. Elections or cancellations of voluntary benefits must be handled directly with the plan (see Voluntary Benefit Plans section below).
Any employee who makes changes must have all supporting documents on file. The deadline for submission of open enrollment elections is October 6, 2017.
***Check your January 2018 paycheck stub to ensure all changes you elected during open
enrollment have been processed. Timely verification of deductions will lessen the
impact to the employee.

- Beginning August 28, 2017 CalPERS subscribers will have access to log in to their my|CalPERS account to view their customized health information and verify health enrollment with 24/7 access to specific health benefit changes that may impact them in the coming year, review a list of enrolled dependents and decide on their health plan options for 2018.
- Western Health Advantage is a new plan partner for 2018 and will provide coverage in the counties of Colusa, El Dorado, Marin, Napa, Placer, Sacramento, Solano, Sonoma and Yolo.
- Kaiser Permanente is expanding its coverage into 13 counties in Washington State for 2018: Grays Harbor, Island, King, Kitsap, Lewis, Mason, Pierce, San Juan, Skagit, Snohomish, Spokane, Thurston and Whatcom.
- Health Net SmartCare is moving into Placer County that includes the cities of Lincoln, Loomis, Roseville, Rocklin and Granite Bay.
- Anthem Blue Cross Select HMO is expanding to Monterey County to replace the Anthem Blue Cross EPO. All Participants and their eligible family members who are currently enrolled in the Anthem Blue Cross (EPO) for Monterey County who do not make a change during Open Enrollment will automatically be moved into the Anthem Blue Cross Select HMO health plan effective January 1, 2018.
- PERS Select, PERS Choice and PERS Care PPO plans will expand the use of Ambulatory Surgery Centers for basic plans to include 12 new outpatient medical procedures, including sigmoidoscopies, tonsillectomies, and kidney stone treatments. A full list of procedures will be listed in each health plan’s Evidence of Coverage. Also added was a site of care program to guide members who need certain prescription drug infusions to lower-cost sites than hospitals (e.g., doctor’s office, ambulatory infusion center, or home infusion). Another addition is a mobile application called Quick Care to help members quickly identify nearby clinics and doctor’s walk-in offices, avoiding the need to use more expensive emergency room care for their urgent care needs.
CalPERS Online Tools
CalPERS has several helpful online tools that employees can utilize to review important
health plan options and information during Open Enrollment. Log in to my|CalPERS to use the Find a Medical Plan tool to research the health plan coverage and benefits
most important to you and your family. Other resources include the Health Plan Search
by Zip Code which is an online tool that identifies which plans are available within a particular
Premiums for dental plans will continue to be fully paid by the CSU, with no cost to the employee. All coverage levels and plan benefits will remain the same for the 2018 plan year.
San Jose State continues to offer both an DHMO and PPO dental plan. Please refer to
the Evidence of Coverage (EOC) booklets for a complete explanation of the benefits
covered within each dental plan, as well as limitations and exclusions that apply.

Vision Services Plan (VSP)
CSU provides basic vision insurance automatically for eligible employee and their eligible family members at no cost to the employee for 2018. The monthly premiums for the basic vision plan will continue to be fully paid by CSU for the 2018 plan year.
Effective January 1, 2018 employees will have the option to purchase VSP Premier coverage, an enhancement to the basic vision insurance plan. The VSP Premier plan offers a higher level of benefits for lenses, contacts and frames. VSP offers a large network of contracting providers including both optometrists and ophthalmologists to choose from when vision care is needed. Both VSP plan options utilize the VSP network of providers, however, out of- pocket costs associated with the plan options will vary. Employees should use the VSP Vision Benefits Summary to determine which plan suits their vision needs. Employees who choose to enroll in the new Premier Vision Plan, may enroll directly with VSP through the VSP website at http://csuactives.vspforme.com or by calling (800) 400- 4569. If an employee chooses to enroll in the Premier plan, any dependents that need to be covered must also be upgraded to the Premier Plan coverage. Employees cannot choose to enroll in both the Basic and Premier coverage at the same time.

If you choose, you can upgrade to the VSP Premier Plan for a small monthly cost. The VSP
Premier Plan allows higher allowances for frames and contacts every calendar year.

Health Care Reimbursement Account (HCRA) & Dependent Care Reimbursement Account (DCRA)
Employees who wish to continue to participate in the Dependent Care Reimbursement Account (DCRA) and/or Health Care Reimbursement Account (HCRA) plan(s) must re-enroll annually during Open Enrollment.
The 2018 annual maximum contribution for the Health Care Reimbursement Account (HCRA) increased to $2,600 ($216.66 per month). The HCRA plan offers participants the ability to pay for eligible out-of-pocket medical expenses with pre-tax dollars. Also, all employees who are currently using the ASI Benny Card will be issued a new ASIFlex visa debit card by January 2018. Employees must submit the Flexible Spending Plan (FSA) Debit Card Request Form (.pdf) directly to ASIFlex to apply for the debit card. The 2018 annual maximum contribution for the Dependent Care Reimbursement Account is $5,000. This plan allows eligible employees the ability to pay for qualifying out-of-pocket dependent care expense with pre-tax dollars. Employees may elect a monthly deduction amount of as little as $20 per month up to a maximum of $416.66 per month. Voluntary Benefit Plans

Please note that all of the voluntary benefit plans below, with the exception of the MetLaw Legal plan, allow employees to enroll or apply throughout the year.
The MetLaw Legal Plan monthly premium is $21.70. MetLaw’s Legal Plan offered by Hyatt Legal Plans, Inc., provides representation for many personal legal services for employees and their eligible dependents. Participants can receive services from a Network or Out-of-Network attorney. Services for covered legal matters performed by a Network attorney are fully paid for by the plan. Out-of Network attorney fees for covered services are reimbursed based on a set fee schedule.

AFLAC offers critical illness insurance which provides payments for certain wellness exams, and a cash benefit if you're diagnosed or treated for a covered critical illness. Please visit AFLAC's website for plan details at AFLAC.
Voluntary Life Insurance
Although Standard does not offer a formal open enrollment period for Voluntary Life
Insurance, employees can enroll in the benefit at any time during employment. If an
employee chooses to enroll after the first 60 days of hire or eligibility, the employee
must submit a completed medical questionnaire along with the enrollment form. Please

Voluntary Long Term Disability Insurance
The Voluntary Long Term Disability Plan administered by The Standard provides loss of income protection (up to 60%) due to a qualified disability. The vendor offers two (2) different plan options which include a 30 or 90 day waiting period. Eligible employees can enroll in the benefit at any time during employment. If an employee chooses to enroll after the first sixty (60) days of hire or eligibility, the employee will be subject to Evidence of Insurability (EOI) requirements. The Auto and Home Insurance plan offered by California Casualty allows CSU eligible employees to obtain auto and home insurance policies at a discounted rate. The plan also has an identity theft component known as ID Defense. All policies are written for a 12-month period. Eligible employees can enroll in this benefit at any time during employment.
