How to Register
There are three steps to registering for the SJSU Staff Olympics
- Someone from your department must complete the Department Registration form
- Each participant must sign the Release of Liability form
- Each participant must pay the $15 registration fee.
If you are not going to participate in any of the events and would like a t-shirt and to attend the Closing Ceremony Barbecue you can skip steps 1 and 2 and go directly to step 3.
Registering your Department
Each department (or group of departments) must submit a Department Registration form (PDF) with the roster completed for the events they are participating in. Only one form is required for each department. Teams have until June 15th to register. Submit hard copies to Julie Wong in Human Resources or email completed forms to julie.wong@sjsu.edu.
We will time stamp all registration forms as they are received. Event brackets will be filled on a first come first served basis. Once the maximum number of teams is reached for each event, no additional teams will be added. Departments are required to designate a team captain who will be responsible for updating their teammates on all Staff Olympics communication. Please include the team captain's email address on the Department Registration form.
Release of Liability Form
All event participants and alternates must sign the Release of Liability form (PDF). The events are non work-related activities and therefore do not qualify for workers' compensation. All participants are participating on a voluntary basis during non-work time (their lunch break). Send all Release of Liability forms to Julie Wong in Human Resources, campus zip 0046.
Payment
Payments can be made by credit card online at the SJSU Staff Olympics eMarket or at Window 1 in the Bursar's Office by cash or check. When paying for multiple people, be sure to get their shirt sizes ahead of time.
All event participants must pay by June 29. Closing Ceremony Barbecue and t-shirt payments will be open until August 10. Shirts are guaranteed to everyone who registers by June 15, after that we will hand them out on a first come first serve basis.
We will check to make sure everyone participating in each event has paid - we will not allow anyone who has not paid or submitted a Release of Liability form to participate...no exceptions!
