Undergraduate Program

The forms and information below apply to students entering the program after spring 08. Those who entered the program prior to fall 08 may choose to remain with the 'old' program or change to the new ones below (strongly encouraged).
  • Hospitality Tourism & Event Management
    • Advising form. This is the form you will use to track your progress and facilitate meetings with your advisor. Note: On the form, IC = Instructor Consent and UD = Upper Division Status.

Undergraduate Program (for those enrolled prior to fall 08)

  • Hospitality Management
    • Advising Form: This is the form you will use to track your progress and facilitate meetings with your advisor. Note: On the form, IC = Instructor Consent and UD = Upper Division Status.
    • Recent Major Form: This is the form you submit (with transcript attached) for approval to your advisor and then the HRTM Chair one year in advance of your intended graduation date.
    • OLD Major Form: Use this on if you entered at a time when there were option areas. This is the form you submit (with transcript attached) for approval to your advisor and then the HRTM Chair one year in advance of your intended graduation date.
  • Recreation
    • Leadership & Administration Emphasis Area
      • Advising Form: This is the form you will use to track your progress and facilitate meetings with your advisor. Note: On the form, IC = Instructor Consent and UD = Upper Division Status.
      • Major Form: This is the form you submit (with transcript attached) for approval to your advisor and then the HRTM Chair one year in advance of your intended graduation date.
    • Park Resource Management Emphasis Area
      • Advising Form: This is the form you will use to track your progress and facilitate meetings with your advisor. Note: On the form, IC = Instructor Consent and UD = Upper Division Status.
      • Major Form: This is the form you submit (with transcript attached) for approval to your advisor and then the HRTM Chair one year in advance of your intended graduation date.
    • Therapeutic Recreation Emphasis Area
      • Advising Form: This is the form you will use to track your progress and facilitate meetings with your advisor. Note: On the form, IC = Instructor Consent and UD = Upper Division Status.
      • Major Form: This is the form you submit (with transcript attached) for approval to your advisor and then the HRTM Chair one year in advance of your intended graduation date.
