Students who have enrolled in one of the HRTM degree lines prior to 2008 may choose to continue on the "old" curriculum pathway or move into the new curriculum. All of the forms below pertain to those students enrolled before 2008 and have chosen to stay on the "old" pathway.

Undergraduate Forms:

Hospitality Management

• Advising Form [doc]: This is the form you will use to track your progress and facilitate meetings with your advisor. Note: On the form, IC = Instructor Consent and UD = Upper Division Status.
• Recent Major Form [doc]: This is the form you submit (with transcript attached) for approval to your advisor and then the HRTM Chair one year in advance of your intended graduation date.
• OLD Major Form [doc]: Use this on if you entered at a time when there were option areas. This is the form you submit (with transcript attached) for approval to your advisor and then the HRTM Chair one year in advance of your intended graduation date.

Recreation

Leadership & Administration Emphasis Area

• Advising Form [doc]: This is the form you will use to track your progress and facilitate meetings with your advisor. Note: On the form, IC = Instructor Consent and UD = Upper Division Status.
