Drivers Form

The SJSU/Pebble Beach Special Event Management Team will have vans that will be driven by team members during Event Week. All team members over 21 years of age will be requested to drive a van.

If you have accepted a position on the SJSU /Pebble Beach Special Event Management Team, and are at least twenty-one (21) years
of age, please read the following requirements and submit the following Designated Van Driver Form:

  1. Show proof of a valid driver’s license
  2. Be willing to provide a DMV driver's history print-out dated no earlier than 14 days prior to January 21, 2012.
  3. Complete, pass and provide proof of The CSU-mandated Defensive Driving Course. (This course can be completed online.)
  4. Pass a drug/alcohol screening to be arranged by Pebble Beach Resorts at Pebble Beach Resorts' cost.
  5. Be at least twenty-one years (21) of age.
  6. Abide by all safety guidelines provided to SJSU by Pebble Beach Resorts. Pebble Beach Resorts reserves the right to disqualify at any time any driver who fails to abide by such guidelines.
  7. Refrain from drinking any alcohol during the days of the event.

 

Please submit the following form no later than:
12:00 PM on Thursday, November 17, 2011




I am not 21 years old or over and am therefore ineligible to be a driver.
