All forms must be COMPLETED and TYPED

You must submit Graduation Application Form, a copy of your unofficial transcript AND a Graduation Major Form  to the department in order to graduate.

  1. If you entered the major starting Fall '14, use this New Major Form.
  2. If you entered the major prior to Fall '14, you may use the Old Major Form or current New Major Form.
