How to Apply for Graduation

All forms must be COMPLETED and TYPED

You must submit to the department in order to graduate:

  1. Graduation Application Form
  2. a copy of your unofficial transcript 
  3. Graduation Major Form (PDF) (Doc
  4. Graduation Minor Form (Doc)

Course Substitution form:

Use this form to for a course that has already been put on Major Form but needs to be changed/updated. Please attache your currrent schedule when submitting the form.
