Public Health Advising
The Undergraduate Program in Public Health offers advising for current students through drop in advising with faculty or peer advisors during the academic year. There is no advising offered during the summer. For urgent matters that cannot wait until the beginning of the fall semester, please email the Public Health Advising Email. This email address will be checked approximately once per week over the summer.

The advising worksheet helps you plan out your courses while you are at SJSU. You can access the forms using the "Forms" tab on the right side of this page.
The major form must be completed when you apply for graduation. The major form is where you list out all the classes that you have completed or plan to complete to meet the requirements of your major. This tutorial explains how to fill out this form. You can access the forms using the "Forms" tab on the right side of this page.
The substitution form is used by students who have already applied to graduate. If your major or minor form lists a course that is different from the course you chose to meet the degree requirement, then you need to complete a substitution form and turn it in to the Registrar's Office. You can access the form from the Registrar's Website.
