Income Tax Filing Information

If you worked or received taxable payments during the 2012 reporting year, you are required to complete and file:

• Federal Form 8843
• Form 1040-NR or 1040-NR-EZ
• Form 540 or 540A for California taxes

If you did not work or receive taxable payments during the 2012 reporting year, you are still required to complete and file Federal Form 8843.

Forms can be downloaded from the following:

• Federal Income Tax Forms 8843 and 1040-NR or 1040-NR-EZ
  (Refer to page 15 for 1040-NR or 1040-NR-EZ; page 69 for Form 8843.)
• California State Income Tax Form 540 or 540A

The Deadline to file 2012 Income Taxes is Monday, April 15, 2013.
