Update Personal Information

Keeping your contact information current is an essential part your academic record.  Use the Personal Information Change Request form [pdf] to update your legal name, address, date of birth, Social Security Number, or provide a gender correction.  

Supporting Documentation

With your change request, you must include copies of supporting documents to validate your new information.  These documents can be items such as a court order, driver’s license, social security card, or passport.

Submitting Your Form

Bring the completed form (along with proper photo ID) to Window "R" in Student Service Center in person.  You may also send your request by mail or fax to 408-924-2077.

Additional Information

Primary Name

  • Legal Name, used for all official university documentation.
  • Will continue to appear on the diploma, official and unofficial transcript, the TowerCard, and any documentation involving financial aid or student accounts.
  • Students must continue to use their primary names when conducting official University business.
  • The student’s SJSU address will continue to use their legal name.
  • Students must use the Personal Information Change form to update their primary name.

Preferred Name Policy
San Jose State University recognizes that members of our community may choose names other than their legal names to identify themselves. SJSU allows preferred names as long as the name is not used to misrepresent or evade legal obligations.  Inappropriate use of the preferred name policy may result in a violation of the Student Conduct Code and be referred to the Office of Student Conduct and Ethical Development. The university reserves the right to remove a preferred name if it is being used improperly.

  • Current students can set up a “preferred name” by changing first and/or middle names on MySJSU
  • The preferred name will only be seen on Class Rosters and Grade Rosters.  All official documents such as transcripts and diplomas must use the student's legal name. 
  • The student’s SJSU.edu email address will remain based on their legal name:  FirstName.LastName@sjsu.edu. 
  • CANVAS does have an internal process that allows you to display your preferred name.
