Change of Name

If your legal name has been changed, you will need to submit a Name Change(PDF)  along with the required documentation to the Office of the Registrar(window "R"). This process is for changing your "Primary Name" on your MySJSU. Please follow the instructions carefully.  There are two ways to submit the request:

1) in person at window “R”(Office of the Registrar), in the Student Service Center(SSC). Please bring your SJSU ID. 

2) mail it directly to the Registrar’s office at:

Change of Address or Contact information

Students can update current contact information by logging into MySJSU or by submitting  a Change of Address/ Contact(PDF) to  window “R”(Office of the Registrar), in the Student Service Center(SSC). Please bring your SJSU ID.
