Change of Name, Address, or Contact Information

Change of Primary Name(Legal Name)

If your legal name has been changed, you will need to submit a Name Change(PDF)  along with the required documentation to the Office of the Registrar(window "R"). This process is for changing your "Primary Name" on your MySJSU. Please follow the instructions carefully. Students will not be able to change their Primary names via MySJSU. There are two ways to submit the request:


1) in person at window “R”(Office of the Registrar), in the Student Service Center(SSC). Please bring your SJSU ID. 

2) mail it directly to the Registrar’s office at:

Setting up of Preferred Name

 

A preferred name is a name the student wishes to be commonly known as, if different from his or her legal first name. Current students can establish a “preferred name” by changing first and/or middle names on MySJSU. Students who are in the process of updating their gender and/or marital status may also change their last names. The student's preferred name will only be seen on Class Rosters and Grade Rosters. Change of Address or Contact information

Students can update current contact information by logging into MySJSU or by submitting  a Change of Address/ Contact(PDF) to  window “R”(Office of the Registrar), in the Student Service Center(SSC). Please bring your SJSU ID.
