In support of student academic excellence and success, the Student Health Center (SHC) also serves as a resource for SJSU faculty.  Currently enrolled students can schedule appointments at SHC for immunizations and physical exams needed to fulfill academic program requirements.

Do the students in your class need a TB screening, immunizations or program physical exams in order to meet academic/internship/program requirements?  The SHC is ready to help.

What can faculty do to facilitate the fulfillment of these requirements for their students? Faculty can:

1. Provide students with the detailed information about the immunization and/or physical exam requirements that they need to fulfill their academic program;
2. Inform students in advance (at least one month) of the due date for completing the requirements;
3. Encourage students to schedule their appointments as soon as possible as the immunization and physical exam appointments are in high demand throughout the semester;
4. Understand that due to privacy and confidentiality laws, we cannot release information about any patient visits.

What do the students in your class need to know to schedule appointments at the SHC to meet their academic/internship/program requirements?  Students need to:

1. Visit the SHC website, http://www.sjsu.edu/studenthealth/online_services/
2. Login to SHC Online Services to complete the required health forms – this needs to happen prior to scheduling an appointment;
3. Call the SHC, (408) 924-6122 or stop by in person (Health Building, 1^st Floor) to schedule an appointment.
4. Bring all required forms to your appointments so that your health care provider can complete them for you in a timely manner.
5. TB screening, immunizations and program physical exams are augmented health care services for which students pay a fee. Students should inquire about fees at the time they schedule an appointment.

What can students do if the available appointments at SHC do not meet their scheduling needs? 

For TB Screening and Immunizations:

• Santa Clara CountyAdult Immunization Clinic  976 Lenzen Avenue, Suite 1500, San Jose.
• The clinic is open Mondays, Tuesdays, Wednesdays and Fridays from 8:15-11:30 am and 1-4pm and Thursdays from 8:15- 11:30 am.

For Physical Exams:

• Primary care physician through their supplemental health insurance coverage
