Forms are intended to be guidelines and to help with "blank page syndrom". No form can fit all programs.

Please add contextual information that will help readers outside your discipline and department understand what you are doing and especially WHY.

• Assessment guidelines spring 2009 through spring 2010 (doc)
• Program report form: alternate for summer 2009 only (doc)
• College summary form summer 2009 (doc) This is for college assessment facilitators to summarize submissions from departments
• SLO centric reporting form (doc) This has the new questions proposed spring 2009. It also has the simplified schedule table. Departments can switch to this wording at their pleasure. The wording of the questions for each SLO seems to speak better to more qualitative disciplines, but may be preferred by any program.

schedule.doc Use this if you want just the new schedule table to substitue in your old form.
