Dissertation Forms and Documents

We are currently switching from APA 6th to APA 7th, and updates to our tips and tricks will be available fall 2020. Please let us know, when submitting a document with APA formatting whether you have used APA 6th or APA 7th as your style guide. YOU MAY NOT COMBINE THE TWO STYLE GUIDES.

It is important that students and faculty check this website each semester for the latest instructions and guidelines. Failure to submit the dissertation as instructed is likely to result in a delay in graduation.

Required Reading

  • SJSU Master's Thesis and Doctoral Dissertation Guidelines (pdf)
    Must be read by all students who plan on submitting a doctoral dissertation to the College of Graduate Studies. These guidelines are effective Fall 2016. Previous versions of the guidelines no longer apply. The SJSU doctoral guidelines also include templates for the introductory pages of your dissertation or dissertation that must be followed. 

Required Forms for Doctoral Dissertations

  • Dissertation Information Packet (pdf) Last updated November 2019. All pages must be filled out and included when you initially submit your dissertation to the College of Graduate Studies for review. The packet includes the following:

  • Dissertation Information Form (pdf)

  • Dissertation Committee Approval Form (pdf) (must be signed by all your committee members at the time you send your dissertation to the College of Graduate Studies). The Dissertation Committee Approval Form serves as evidence that your committee members have all approved your dissertation. The dissertation itself should contain only a dissertation committee page that lists your committee members and follows the template provided in Appendix E of the dissertation guidelines.

  • SJSU License AgreementUpdated to include several different embargo options pursuant to University Policy S14-10. Please fill out the dissertation information packet completely. The dissertation information packet must be included as a separate attachment when you email your dissertation for initial review to the College of Graduate Studies (i.e., do not incorporate the forms into your dissertation document). 

Digital or handwritten signatures are accepted from faculty members on the Dissertation Committee Approval Form and from the student on the SJSU License Agreement.

Required Supplementary Documents (When Applicable)

  • IRB or IACUC Approval Letter (if applicable)
    If research for your dissertation involved either human or animal subjects, you must provide evidence that you had obtained the appropriate permission before conducting your research and collecting any data. Approval is needed from either

    The SJSU Institutional Animal Care & Use Committee (IACUC)

    IRB or IACUC approval letters should be included as a separate attachment when you email your dissertation for initial review to Graduate Studies.

  • Copyright Permissions (if applicable)
    If copyrighted material is included in your dissertation, you must obtain permission from the copyright owner and provide evidence that such permission was obtained at the time that you submit your dissertation to the College of Graduate Studies. Examples of copyrighted material may include any images that are not your own - tables, figures, graphs, photographs, maps - as well as extensive portions of text, such as the reproduction of journal articles.
