On May 18, 2021, the Santa Clara County Health Officer announced that all businesses and governmental entities in the county are required to determine the COVID-19 vaccination status of their employees (as well as contractors, volunteers, and others who regularly work onsite). Under the Health Order, employers are required to ask all personnel for their vaccination status and maintain appropriate records to demonstrate compliance with the Order. Until a person’s vaccination status is ascertained, they must be treated as unvaccinated. Employees may decline to provide vaccination information, in which case the employee’s status will be recorded as unvaccinated.

As required, we are asking all SJSU and auxiliary employees, student employees, volunteers, unpaid interns, and unpaid visiting scholars to complete the Self-Certification of Vaccination Status form no later than 11:59 p.m. on Tuesday, June 1, 2021, the county deadline for initial ascertainment. This form collects basic information and asks you to accurately describe your vaccination status. If you are fully vaccinated you will only be required to complete this form one time. For purposes of the Health Order, fully vaccinated means it has been at least two weeks since you received the second dose of the Pfizer or Moderna vaccine, or the single dose of the Johnson & Johnson/Jansen vaccine.

If you are not fully vaccinated, decline to provide vaccination information, or do not respond in any manner, the request to complete this form will be sent to you every 14 days to collect your current vaccination status. Until a person’s vaccination status is self-attested as “fully vaccinated,” they will be considered unvaccinated. Individuals who decline to provide vaccination status or do not respond will also be considered unvaccinated.

Regardless of your vaccination status, SJSU’s mask and face covering guidelines as part of the SJSU Adapt plan remain the same at this time. Masks or face coverings must continue to be worn at all times on campus, including outdoors. The only exception is if you are alone in a private office with the door closed. SJSU will continue to monitor the evolving mask and face covering guidelines and will update the campus community of any changes as we look ahead to the fall semester.

Remember, getting vaccinated is the best thing you can do to keep yourself safe from COVID-19 and prevent you from spreading COVID-19 to your family, friends, coworkers, and neighbors.
