- I have changed my address in PeopleSoft. Do I need to do anything else?
- I changed my primary name (legal name). What do I need to do in order to change it
with the University system?
There are two things that are needed in order to process a name change with the University system. The appropriate form listed below and your original social security card that indicates your new name.
- An original Social Security Card indicating the name change must be shown.
- Complete the appropriate form.

Complete all appropriate areas for an Employee Action Request [DocuSign] form.
For Student Assistants
Complete the Student Personal Information Change Request Form.
