Full- and part-time permanent State employees are reviewed annually using the form below. The annual review cycle covers the performance review period of July 1 through June 30 of each year. This is intended to be a comprehensive review of your performance during the prior year and is designed to provide a vehicle for on-going feedback and dialogue. In addition, it acknowledges strengths and core competencies.
Employees who are new to the Management Personnel Plan (MPP) are evaluated after six months and again at one year of service, and thereafter annually under the campus annual review program.
Annual evaluations should be completed and returned to University Personnel no later than 45 days after the end of the evaluation cycle (July 1 through June 30). Six-month evaluations should be completed and returned to University Personnel no later than 14 days after the employee’s sixth month of service.
