  The following forms are to be completed after being accepted to the program:

• Participant Agreement Form
• Health Insurance Enrollment Form: The cost of the health insurance is included in the total cost of each program. You will therefore need to fill the form and submit it to the study abroad office. We recommend that you also download the complete policy brochure (6 MB) or pick-it-up from our office. for more information about the policy visit the CSU health website.
• Certification of Health Form
• Independent Travel Release Form Only for students that will be traveling independently to or from the program site.
