Web Requisition Instructions
- Complete the end user training.
- Contact the CMS Help Desk at 408-924-1530 to schedule a one-on-one training.
- Enter a web requisition in the SJSU Financial Transaction Services.

- Any requisition support documentation should be uploaded in Financial Transaction Services when initally submitting the web requisition. For help uploading support documentation, please contact the CMS Help Desk at 408-924-1530.
- You may also save supporting documentation as a pdf attachment and email it to pass.req@sjsu.edu. In the subject line of the email, reference only the complete requisition number (10 characters and in caps).
- You may also fax the documents to 4-6684. The resolution on your fax machine should be set to HIGH or FINE. Legibly write the complete requisition number (in caps and a total of 10 characters) in a prominent location of the first page of your backup submission. Each fax transmission (call) you make must pertain to one requisition.
- Do not send hardcopies of your supporting documents through intercampus mail or email them directly to the buyer. They will be returned to you for proper submission.
- Have your approver electronically approve the requisition.
- Status will be approved.
- The next day, status will be Distributed.
- Check the status of your order:
- For status of your web requisition, run a PO Life Cycle Report.
- For status of the purchase order, check the Purchasing Directory and contact the buyer listed on the purchase order.
A Note About Chartfield Changes
When processing purchase orders, buyers must enter a commodity code that correctly reflects the product and/or service being ordered. If necessary, the system will automatically change your chartfields to reflect the correct account number associated with the commodity code entered.
Purchasing will not change Chartfields once a purchase order is created in the system. For information on commonly used account codes, please see Account Definitions on the Accounting Services - Account Definitions website.
