Medical EmergenciesInjury and illness are among the most common campus-related emergencies. San José State maintains a Student Health Center on campus where students can be seen at any time for any medical reason. The Student Health Center will also see staff and faculty for initial evaluation of any on-the-job workers compensation injuries. Subsequent medical care for staff and faculty for on-the-job injuries is provided by U.S. HealthWorks, a nearby health facility. Emergency treatment can be obtained at O'Conner Hospital for all serious injuries and injuries or illnesses incurred after regular work hours. Medical/Injury DocumentationIn accordance with workers' compensation laws, an employee or campus volunteer’s supervisor must provide injured employees with an employee's claim for workers' compensation benefits as soon as possible within 24 hours of the incident. Employees should follow this protocol:
Car CrashesAll on-campus car crashes resulting in injury must be reported to the University Police. All off-campus car crashes involving SJSU vehicles or private vehicles driven by university personnel while on university business are subject to local law enforcement rules and should be reported to the jurisdiction where the accident occurred. The driver of a university or state vehicle that is involved in a crash must record all pertinent information on the Accident Identification Card, Standard Form 269, before leaving the scene of the crash. If another vehicle is involved, the driver of that vehicle will be given the appropriate portion of Form 269. All crashes involving university vehicles or private vehicles driven by university personnel that result in injury to any person or that involve serious damage to private property must be reported immediately to the California Office of Insurance and Risk Management at (916) 445-2184. The driver of the state vehicle must complete and submit the Report of Vehicle Accident Form, Standard Form 270 within 48 hours of the accident. If that person is unable to complete the form, then the immediate supervisor or person who authorized the employee to use the vehicle should ensure that the form is completed and inform the Office of Insurance and Risk Management. Return to Normal OperationsIf the incident has resulted in the evacuation of part or all of a campus building(s) then those areas will remain closed until the University Police Incident Commander determines:
