The SJSU thesis guidelines also include templates for the introductory pages of your thesis that must be followed.
Thesis Information Form (pdf)
Must be filled out and included when you initially submit your thesis to Graduate Studies and Research. You do not need to have your committee members sign this form; simply print the committee members' names.
Thesis Binding Checklist (pdf)
Must be read by all students whose thesis has been approved by Graduate Studies and Research and who are submitting final copies for binding.
ProQuest/UMI Publishing Agreement (pdf)
Must be filled out, signed, and included with the final copies of the thesis for binding, along with the appropriate fees. Instructions on how to fill out this form are provided in the Thesis Binding Checklist.
Please read this agreement thoroughly and only submit the embedded forms AFTER your thesis has been approved by Graduate Studies and Research as instructed on the Thesis Binding Checklist; you may retain the informational pages for your records.
