Required Documents for Form Submission (any form)

All students requesting signature from the Chair or Major Advisor need to bring the following documents along with the completed form:

1) "My Academics" Homepage (print screen)
2) Course History (via "My Academics")    
3) Unofficial transcripts from SJSU  
4) Unofficial transcripts from previous college(s) OR print a copy of the "Transfer Credit" report from MySJSU "My Academics" section

Note:

  • All the above mentioned required documents will not be returned to the student. They will be kept by the department.

  • Please allow 3-4 business days of processing time and check back with the office for the status of the paperwork.

  • Students are supposed to get the forms printed and the department does not give out or print forms.

  • Please read the instructions on the forms carefully.

  • Only the Graduation application and Substitution form will be sent over to the Registrars at SSC (Students Services Center) directly by the department when you submit the form. For all other forms please follow the instructions on it.

TO ENSURE YOU HAVE THE LATEST VERSION OF THE FORM, IT IS BEST TO DOWNLOAD ANY FORM WHICH BELONGS TO THE REGISTRARS THROUGH THEIR WEBSITE. SUCH FORMS ARE MARKED WITH "[R]"

  • Change of Major for students fewer than 90 units [pdf] [R]

    [REVISED!] Use to declare new, change or add (double) Major or Minor. Requires the signatures of the Advisor or Chairperson of your new Major/Minor Department. Change of Major to undeclared, requires signatures of GE Advisor and Director of Academic Advising and Retention Services. Submit completed form to the "R"(Registrar) counter at the Student Services Center.

If you are a student with 120 units or greater, you need two additional signatures from the College Associate Deans and Director of Academic Advising and Retention Services. Change of Major to undeclared, requires signatures of GE Advisor and Director of Academic Advising and Retention Services.     Change of Graduate Major[pdf] For Graduate Students [R]

    NOTE:Students requesting signature from the chair, please bring the required documents along with the Change of Major/Minor Form.

  • Excess Units Petition [pdf] [R]

    To be used by students who wish to enroll in more that 18 units. Print and read the form carefully for instructions and qualifications. Submit this form to the Academic Advising and Retention Services(AARS) from the first day of instructions to the last day to add(add deadline).

  • Graduation Application

    Please read the instructions on the application carefully. Get the signature of Major Advisor where necessary and submit the completed application to the Department of Sociology along with your major form and the other required documents.

    NOTE: Students requesting signature from the chair, please bring the required documents along with the Graduation Application, Contract for Major Form and Major Advisor's signature on both.

  • Graduation Date Change [pdf] [R]

    If you have submitted a graduation application but will not complete your requirements by the graduation date you applied for. You must meet with your major advisor for the review and approval. There is a $10 fee that must be paid to the Bursar's Office before submitting the completed form to window "R" (Registrar) in the Student Services Center(SSC).

  • Late Drop Request [pdf] [R]

    Use this petition to drop one course during the current semester or retroactively. Please use a separate petition for each course you wish to drop. Although this petition notes the current semester's date, it can still be used for retroactive drops from a previous semester. Please read instructions carefully and submit completed request to Academic Advising and Retention Services(AARS). If you wish to drop all your classes, please use

  • Open University Forms

    Please visit the Open University website to download the Open University Registration or Drop forms. Read forms carefully for instructions and deadlines.

  • Reinstatement Petition [pdf] [R]

    Use to clear academic disqualification status. Read reinstatement guidelines carefully and meet with an advisor prior to submitting.

  • Retroactive Graduate Petitions

    Retroactive and late course and semester withdrawals, which supersede the former Retroactive Course Drop and Retroactive Semester Withdrawal forms, are now combined into a single form, the Graduate Petition for Course/Semester Withdrawal, available at http://www.sjsu.edu/gape/docs/withdrawal.pdf.

  • Retroactive Undergraduate Petitions

    Undergraduate retroactive petitions include Retroactive Academic Renewal, Retroactive Add or Drop, Retroactive Withdrawal.

  • Sociology 100W Credit Request [pdf]

    Submit the completed form to the Sociology Office, DMH 241.

  • Sociology Contract for Major [pdf]

    Please Use ink. Identify courses taken at other schools by writing that school's course # and the name of the school. Submit the completed major form along with your application to the Department of Sociology, DMH 241.

    NOTE: Students requesting signature from the chair, please bring the required documents along with the Graduation Application and Contract for Major Form.

  • Transcript Request

    Use to request official copies of your SJSU academic transcript. Please read instructions and regulations on the request form.

  • Withdrawal from the Semester Request [pdf] [R]

    To be used after the drop deadline and submitted before the last day of classes. Use only for documented extenuating circumstances. Read instructions carefully and submit completed request to the Academic Services Office. This request is to drop all your classes for the current semester.
