To request the following:
- a message to be sent to your provider, call (408) 924-6122, during hours of operation.
- schedule, reschedule, or cancel an appointment visit the SHC Online Services or call (408) 924-6122 if the reason for your appointment is not listed online;
- a copy of your medical record, download and complete the Medical Records Request Form and fax to SHC;
- an itemized statement, download the Itemized Statement Form (augmented health care fees you paid).
General questions will be answered within 10 business days.
