The Alcohol e-CheckUpToGo is an interactive web survey that allows college students to enter information about their drinking patterns and receive feedback about their use of alcohol. The assessment is self-guided and will provide you with a personal assessment of your alcohol intake.

Instructions for Sending Proof of Completion

Step 1: Click on the verify completion of your e-CheckUpToGo button on the last page of the survey.

Step 2: Enter the following information for the Electronic Verification of Completion program:

•    Your name 
•    Your email address
•    San Jose State University ID number

Step 3: Submit your electronic verification to SJSU Alcohol Ed.
