Injury & Illness Prevention Program
Each department manager is responsible for adhering to the University Illness & Injury Prevention Program (IIPP).

The program includes the following elements:
- Motivation and Discipline: Your department needs a system to train employees in safe and healthy work practices. You can use incentives, training and retraining or disciplinary action.
- Communication: Use meetings, training programs, postings and SafetyWorks Newsletter articles to inform your employees of the IIPP. Encourage employees to report hazards within the work site without fear of reprisal.
- Hazard Identification and Awareness: Evaluate workplace hazards through periodic scheduled self-inspections. Use our General Safety Inspection Checklist (PDF) to assist you.
- Accident Investigation: Accident investigation is an important part of San José State University’s safety program. It helps prevent future similar accidents. Follow our Occupational Injury or Illness Reporting Policy (PDF)
- Hazard Correction & Documentation: You are responsible for correcting unsafe conditions discovered during inspections or reported by employees.Use our Hazard Assessment Survey & Analysis Personal Protective Equipment (PPE) form (.doc) to assist you.
- Training: As a manager, you need to know the hazards your employees face. You are responsible for making sure your employees are trained to perform their work without illness or injury. All SJSU employees need to attend Required Safety Training.
- Documentation: Keep safety training, inspection and accident investigation documents in a centralized file. You may need to show them to inspectors. Remember, if you don’t have the paperwork, it didn’t happen.
