The Catastrophic Leave Donation (CLD) Program allows for the donation and receipt of vacation and sick leave credits between eligible university employees, regardless of bargaining unit.

If you have a qualifying catastrophic health condition (your own or that of your spouse, domestic partner, parent or child), you should consult with your Benefits Services Representative.

• Catastrophic Leave Medical Certification (PDF): Used by your health care provider (or your family member's health care provider) to certify medical condition.
• Donation Program Information Sheet (PDF): Provides information about the benefit program and application process for catastrophic leave.
• Catastrophic Leave Donation Request (PDF): Used to submit donations of sick or vacation leave to employees who are eligible for catastrophic leave.
• Catastrophic Leave Donation Program Participation Request (PDF): Used to apply for catastrophic leave benefits.
