Visitor Accidents & Filing A Claim Against SJSU
In the event of an accident or injury involving a visitor to the university, the highest priority must always be caring for the individual.
- Immediately use a blue light phone or call the University Police Department at 924-2222 and follow their instructions.
After the visitor has been cared for and the accident is under control, report the incident to University Risk & Compliance:
- Review our Visitor Accident Guidelines
- Complete our Visitor Accident Report form
The Visitor Accident Report is necessary to assist the University in proper investigation and follow-up measures to prevent further accidents and to assist with the injured person's own medical insurance. It is needed in addition to any reports that UPD or other police agencies may file.
Information On Filing A Claim Against San Jose State University
All claims against San Jose State University must be filed with the Office of Risk Management and Public Safety within six (6) months of the accident or incident date of occurrence. Completed claim forms must be mailed or delivered to the address below along with a $25 Claim Filing Fee, checks are to be made out to "Trustees of CSU."
