Required Documents for Form Submission

All students requesting signature from the chair need to bring the following documents along with the completed form:

  • "My Academics" Homepage (print screen)
  • Unofficial transcripts from MySJSU
  • Course History (via "My Academics")
  • Unofficial transcripts form previous college(s)[i.e. community college] or print a copy of the "Transfer Credit" report from MySJSU "My Academics" section

Note: Please allow 3-4 business days of processing time and check back with the office for the status of the paperwork.

 

  • Change of Major or Minor [pdf]:
    [REVISED!] Use to declare new, change or add (double) Major or Minor. Requires the signatures of the Advisor or Chairperson of your new Major/Minor Department. If you are a student with 120 units or greater, you need two additional signatures from the College Associate Deans and Director of Academic Advising and Retention Services. Change of Major to undeclared, requires signatures of GE Advisor and Director of Academic Advising and Retention Services. Submit completed form to the "R"(Registrar) counter at the Student Services Center.

    Students requesting signature from the chair, please bring the required documents along with the Change of Major/Minor Form.
  • Change To Graduation Contract [pdf](Substitution Form)
    Submit the completed form to the Department of Sociology, DMH 241.
  • Excess Units Petition [pdf]To be used by students who wish to enroll in more that 18 units. Print and read the form carefully for instructions and qualifications. Submit this form to the Academic Advising and Retention Services(AARS) from the first day of instructions to the last day to add(add deadline).
  • Graduation Application [pdf]
    Please read the instuctions on the application carefully. Submit the completed application to the Department of Sociology along with your major form and the other required documents.

    Students requesting signature from the chair, please bring the required documents along with the Graduation Application and Contract for Major Form.
  • Graduation Date Change [pdf]
    If you have submitted a graduation application but will not complete your requirements by the graduation date you applied for. There is a $10 fee that must be paid to the Bursar's Office before submitting the completed form to the Registrar (window "R" in the Student Services Center).
  • Late Drop Request [pdf]
    To be used after the drop deadline and submitted before the last day of classes. Use only for documented extenuating circumstances. Read instructions carefully and submit completed request to the Student Advising Center. Note: if your intention is to drop all your classes for the current semester, please use a Withdrawal from the Semester Request (PDF).
  • Open University Forms
    Please visit the Open University website to download the Open University Registration or Drop forms. Read forms carefully for instructions and deadlines.
  • Reinstatement Petition [pdf]
    Use to clear academic disqualification status. Read reinstatement guidelines carefully and meet with an advisor prior to submitting.
  • Request for Sociology Statistics Credit [pdf]
    Submit the completed form to the Sociology Office, DMH 241.
    Note: Only courses that are articulated with Soci 15 or Soci 102 will be accepted. Please check the SJSU Articulation for updated information.
  • Request for Sociology 100W Credit [pdf]
    Submit the completed form to the Sociology Office, DMH 241.
  • Retroactive Graduate Petitions
    The Graduate Retroactive Course Add form is available at http://www.sjsu.edu/gape/docs/course_add.pdf. Retroactive and late course and semester withdrawals, which supercede the former Retroactive Course Drop and Retroactive Semester Withdrawal forms, are now combined into a single form, the Graduate Petition for Course/Semester Withdrawal, available at http://www.sjsu.edu/gape/docs/withdrawal.pdf.
  • Retroactive Undergraduate Petitions
    Undergraduate retroactive petitions include Retroactive Academic Renewal, Retroactive Add or Drop, Retroactive Withdrawal.

  • Sociology Contract for Major [pdf]Please Use ink. Identify courses taken at other schools by writing that school's course # and the name of the school. Submit the completed major form along with your application to the Department
    of Sociology, DMH 241.

    Students requesting signature from the chair, please bring the required documents along with the Graduation Application and
    Contract for Major Form.
  • Sociology Contract for Minor[pdf]
    Submit the completed minor form to the Department of Sociology, DMH 241.

    Students requesting signature from the chair, please bring the required documents along with the Contract for Minor form.
  • Transcript Request [pdf]
    Use to request official copies of your SJSU academic transcript. Please read instructions and regulations on the request form.
  • Withdrawal from the Semester Request [pdf]
    To be used after the drop deadline and submitted before the last day of classes. Use only for documented extenuating circumstances. Read instructions carefully and submit completed request to the Academic Services Office. This request is to drop all your classes for the current semester.
