The CSU provides Family Care and Medical Leave to eligible employees in accordance with the Federal Family and Medical Leave Act (FMLA), the California Family Rights Act (CFRA) and Collective Bargaining Unit Agreements.

Upon completion of FMLA, you are entitled to return to the same position or another position with equaivalent benefits, pay and conditions of employment.

You are also entitled to retain any health insurance benefits during time away from work due to FMLA.

• Family Medical Leave Department (FML) Checklist (PDF): For your department manager to document your medical leave.
• Family Medical Leave (FML) Information Sheet: Provides information about eligibility, leave status, health benefits, reinstatement rights, and the application process.
• Leave of Absence Request Form (PDF): Your request for leave of absence.
• Employee Medical Certification (PDF): Used by your health care provider to certify your health condition and time away from work.
• Family Medical Certification (FML) (PDF): Used by your family member's health care provider to certify their health condition and your time away from work.
• Return to Work Certification (PDF): Health care provider certifies your release to return to work.
