Accident Reporting

In the event of an accident or injury on campus, the highest priority must always be caring for the individual.

If your injury is serious, call 911 or contact the University Police Department at 924-2222 and follow their instructions.
Students representative, instructor or other, shall complete the Student Accident Report (PDF).
If you are a student employee, immediately report any work-related injury or illness to your supervisor. See Workers’ Compensation for more information.
If you can walk, have someone walk with you to the SJSU Student Health Center.
