Required Special Session Forms

Special session forms are mandated. All forms must be completed and submitted to CPGE by intercampus mail (extended zip 0135) or fax (408-924-2666).

  1. Instruction Appointment Form (PDF)
    Used to appoint faculty for special session and winter session.
  2. Class Section Add Form (PDF) 
    Used to schedule or add a new course.
  3. Class Section Change Form (PDF)
    Used to update or make changes to an existing course.
  4. Pay Voucher Form (PDF)
    Used by faculty to receive payment.
