How to Request a Unitrack Partnership

Begin by filling out the Unitrack Partnership Interest Form. 

Once the departments approve the Unitrack Partnership, please complete the following steps (although you can begin these steps prior to department approval): 

  1. Make a copy & fill out the "SJSU UniTrack Course Request Template". 
  2. Make a copy & fill out the "Unitrack Syllabus Template". 
  3. Make a copy & fill out the "Unitrack Lesson Plan Template"
  4. Submit these documents to the Unitrack Partnership Document Submission google form.
