SJSU Immunization Requirements

FALL 2020 NEW STUDENTS

New students should provide proof of  immunization prior to the first day of classes. 

Students who need to get these vaccinations or gather proof of immunization are strongly encouraged to do so now.  Acceptable proof of immunization:

  • can be obtained from the student’s immunization record, including the name and date(s) of immunization or date and results of blood testing indicating immunity;
  • may be provided by the student’s high school, private health care provider or local health department.
Students will be able to upload their documents through the immunization portal link below beginning May 1, 2020.  Paper submissions cannot be accepted.
 
Please note: We are currently experiencing a delay in the review/verification of immunization compliance records that have been submitted. This delay will not impact your ability to register for classes. We greatly appreciate your understanding and your patience.
 
New students who need to meet the immunization requirements will be notified via an email from the Student Health Center.

 

Required Immunizations

  • Measles and Rubella (MMR) Vaccine - Series of 2 vaccinations started after one year of age and at least one month apart. You must submit proof of immunization if you were born after January 1, 1957.
  • Hepatitis B Vaccine: Series of 3 vaccinations, which may take up to seven months to complete (or series of 2 vaccinations of newer Heplisav vaccine for anyone 18 years and older). All incoming students who are 18 years of age or younger on the first day of their first term must submit proof of Hepatitis B immunization.  

 

Strongly Recommended Immunizations:  

  • Mumps
  • Meningococcal conjugate: (Serogroups A, C, Y and W-135) 1 dose on or after age 16 for all students 21 or younger.
  • Tetanus/Pertussis/Diphtheria Booster (Tdap): all students should have had 1 dose of Tdap after age 7.
  • Varicella (Chicken Pox Vaccine): 2 separate doses for all who have not had natural chickenpox.

* Exemptions - Students may be granted medical exemptions from required immunizations if they can provide a signed statement from a physician documenting either: a) the student has a physical condition or medical circumstance that contraindicates vaccination. b) other evidence of immunity. Duration of exemption (permanent or temporary with end date) should be specified.

Note: Any student without documented proof of immunity may be asked to leave the residence halls or campus during an outbreak of an indicated infectious disease.
