San Jose State University is closely monitoring the Novel Coronavirus (COVID-19) pandemic and we are committed to keeping the campus community safe.

The Report a Case form provides a confidential space where information can be collected and shared with only the necessary campus officials.

Whether studying or working from home or participating in on-campus activities, students, faculty, and staff are encouraged to report their positive case so that support resources can be extended to them. 

As a member of the Spartan community, you should use this form if you have experienced the following:

  • Have been in close contact with someone who has tested positive
  • Have symptoms of COVID-19 and am awaiting a test.
  • Have tested positive for COVID-19, even if you aren’t exhibiting symptoms.
  • Have been directed to complete this form after using the SJSU self-screening tool.

Report a COVID-19 Case

 

The SJSU COVID-19 Dashboard provides information on confirmed cases in the past 14 days, known cases in quarantine or isolation, the cumulative confirmed case count for the university, and Santa Clara County statistics. The dashboard is updated regularly.
